Vaccine Eligibility Updates

Please take two minutes to complete the survey found at: https://www.surveymonkey.com/r/GPHCOVID19Vaccine. We will be matching your new response with your old one so we can use your original submission date and you will “not lose your place in line”. The survey should only take about 2 minutes and your eligibility status may have changed since the time you completed it the first time. We are prioritizing by age first and then by date of submission.

Geauga Public Health will soon be launching a web-based tool to actually schedule an appointment. Until that is ready, we will be using this information to call out to schedule appointments.
